2012 Top Hands Medical History Physicians Statement

PLEASE READ THIS MESSAGE CAREFULLY.  If your athlete(s) is at least 8 years of age by February 15, 2012 AND you wish them to participate in the Top Hands Horse Show which is February 17-19, 2012, I need you to complete the attached form by taking it to a physician of your choice, as soon as possible and NO LATER THAN December 31, 2011.

Once signed by a physician, give the form to me, please.

The Houston Livestock Show and Rodeo has made this form MANDATORY and requires that it be completed within the 12 month period prior to February 17, 2012.

My apologies for getting this to you so late, they just sent us to the site to get this form this afternoon.

At your next lesson, you will fill out their liability release but NOW, I need you to act as soon as you are financially and logistically able.
